Emergent epidemiological trends and new organizational modes for Health Care Systems
Epidemiological emergent trends: 30% of the population has chronic diseases and 4% of the population is in need of LTC. Chronic diseases effective treatment needs patients early enrolment, inter-professional convergences about treatment paths, patients’ compliance control. None of this tasks can be fulfilled by traditional acute care hospitals. Also LTC asks for new provision modes because nursing homes for 2,3 milion elderly are not affordable and the actual home care intensity is ineffective. How can we redesign the service landscape to be consistent with the emergent epidemiological trend? How do we need to reallocate resources? Which are the best change drivers?
